Analysis of pharmacist‐identified medication‐related problems at two United Kingdom hospitals: a prospective observational study

Study (1503 admissions) suggests patients are at greatest need of pharmacist input in terms of clinically relevant medication‐related problems (MRPs) during early stages of inpatient stay (2614 validated MRPs identified, of which 1153 moderate or severe, and preventable).
