Comparison of Cardiovascular and Safety Outcomes of Chlorthalidone vs Hydrochlorothiazide to Treat Hypertension

Cohort study (n=730,225) found no significant difference in the risk of myocardial infarction, hospitalisation for heart failure, or stroke between chlorthalidone and hydrochlorthiazide (HR 1.0, 95% CI 0.85-1.17).

SPS commentary:

Chlorthalidone was associated with a higher risk of hypokalaemia (2.72, 2.38-3.12), hynonatraemia (1.31, 1.16-1.47), acute renal failure (1.37, 1.15-1.63), chronic kidney disease (1.24, 1.09-1.42) and type 2 diabetes (1.21, 1.12-1.30), whilst chlorthalidone was associated with a higher risk of abnormal weight gain.
