Cumulative Incidence of Thiazide-Induced Hyponatremia: A Population-Based Cohort Study

Danish target trial emulation study (n=180,476) found increased risk of hyponatraemia <30 days from initiation of bendroflumethiazide vs calcium channel blocker (HR 3.56;95%CI 2.76-4.60) & hydrochlorothiazide+renin-angiotensin system inhibitor(RASi) vs RASi alone(4.25;3.23-5.59)

SPS commentary:

Researchers conclude from these findings that treatment initiation with thiazide diuretics suggests a more substantial excess risk for hyponatremia, particularly during the first months of treatment, than indicated by US drug labelling.
