Development and validation of a ready-to-use score to prioritise medication reconciliation at patient admission in an orthopaedic and trauma department
Review of 888 patients found 387 unintentional discrepancies, 65.1% were drug omissions. 5 independent predictors of unintentional discrepancy were identified, age >75 years, admission during school holidays, female gender, emergency hospitalisation and being on ≥5 medications.
