Effect of postoperative continuation of antibiotic prophylaxis on the incidence of surgical site infection: a systematic review and meta-analysis

Review of 52 RCTs (n-19,273) found no significant decreased risk of infection with immediate discontinuation vs prolonged antibiotic prophylaxis (RR 0.89, 95% CI 0.79-1.00).

SPS commentary:

In those trials that met best practice standards for antibiotic prophylaxis, there was no benefit of extended prophylaxis (RR 1.04, 95% CI 0.8-1.27), whereas in those trials which did not adhere to best practice standards, prolonged prophylaxis reduced infection rates (0.79, 0.67-0.94).  Authors state that these findings support WHO recommendations against this practice.
