Retrospective analysis (n=4677; 82.2% treated with glargine and 17.8% with detemir) found excess risk for hypoglycaemia in patients with hypoalbuminaemia on detemir, but there was no difference in occurrence of hypoglycaemia between two insulins in those with albumin > 3 g/dL.

The researchers say that these findings support the long-held belief that insulin detemir levels are less predictable in hypoalbuminaemia; and as there was no difference in occurrence of hypoglycaemia in individuals with albumin over 3 g/dL, this confirms the equivalent safety of both insulins in the majority of people.
