Summary
This study reports an increased risk of lactic acidosis/ elevated lactate concentrations in metformin users who have a renal function <60mL/min/1.73m2 (adjusted HR 6.37; 95% CI 1.48–27.5), particularly in users ≥730g during the preceding year and/or a recent high daily dose (>2g)

The results of this study are consistent with current recommendations (as per Glucophage® SPC) that renal function of metformin users should be adequately monitored, with use contra-indicated in those with renal failure or dysfunction (creatinine clearance <60mL/min).
