Summary
RCT in 402 Asian patients found that a premixed insulin lispro regimen thrice-daily was non-inferior to basal-bolus therapy in terms of overall glycaemic control (HbA1c change at week 24 was −1.1% for both groups). Frequency of adverse events was comparable.

A commentary notes that though premixed drugs offer convenience, separate drugs enable flexibility and a more physiological treatment, thus, to mix or to separate will still be an important clinical question with the advent of newer injectable anti-diabetic drugs, and different approaches need to be tested in a population-specific manner.
