Summary
Clinical resolution to day 28 (primary endpoint) was achieved in more women (n=513) with uncomplicated urinary tract infection, treated with 5-day nitrofurantoin vs single dose fosfomycin (70% vs 58%; p= 0.004). Microbiological cure was also more favourable (74% vs 63%; p=0.04).

A related editorial notes that the optimal nitrofurantoin dosing regimen and whether current recommendations of 100 mg twice daily (licensed dose) are equally effective as 100 mg 3 times daily (dose used in trial) remains unknown. The role of fosfomycin also requires examination; in particular alternative prescribing practices for multidrug-resistant uropathogens should be investigated.
