FDA cautions about dosing errors when switching between different oral formulations of antifungal Noxafil (posaconazole)

This alert advises caution when switching between the posaconazole oral suspension and the tablet, highlighting that direct mg for mg substitution of the two formulations can result in drug levels that are lower or higher than needed to effectively treat certain fungal infections

The FDA advises that prescribers should specify the dosage form, strength, and frequency on all prescriptions they write for Noxafil. 

 

In the UK, the SPCs for Noxafil tablet and oral suspension specify that these products are not to be used interchangeably due to the differences in the dosing of each formulation.
