Evaluating the impact of a pharmacist-led prescribing feedback intervention on prescribing errors in a hospital setting
Audit (5191 prescribed medications at baseline; 5122 post-intervention) found pharmacist-led, individualised constructive feedback on prescribing (PS) reduced PS errors vs. existing practice (change in error rates 23.7%; p<0.05) & potential to improve PS outcomes & patient safety
