Medication-related harm in New Zealand general practice: a retrospective records review
This study (n=9076) found the incidence of medication-related harms was 73.9 per 1000 patient-years (1 in 5 moderate or severe); 15.6 per 1000 patient-years were (potentially) preventable. Cardiovascular, antineoplastic/immunomodulatory agents and anticoagulants caused most harm.
