Healthcare Safety Investigation Branch (HSIB) launches investigation into weight-based medication errors in children

The HSIB will look at the risks involved when prescribing, dispensing and administering medicine to children, after it was notified of an event involving a 4-year-old child, where errors in various processes meant they received 10 times the intended anticoagulation dose.
