Hospital discharge service: policy and operating model

This document sets out how health and care systems can ensure that people are discharged safely from hospital to the most appropriate place and continue to receive the care and support they need. It replaces COVID-19 hospital discharge service requirements published 19/3/20.

This guidance is supported by:

- Action cards which set out how roles/responsibilities of health/care staff (doctors; matron and ward manager; acute therapy teams; bedded rehabilitation staff; social care staff; CCGs and commissioners; managers/members of discharge team) will change in line with hospital discharge service.

 

- Various patient information leaflets regarding hospital discharge.

 

- A £588 million fund to cover 6 weeks of adult social care or the immediate costs of care in their own home.
