Intranasal vitamin B12 administration in elderly patients: A randomized controlled comparison of two dosage regimens
RCT (n=60,age ≥65 years) found both the loading dose regimen (1000 mcg daily for 14 days then weekly) and no loading regimen (1000 mcg every 3 days for 90 days) resulted in a rapid increase of vitamin B12 and holotranscoblamin concentrations.

SPS commentary:
Based on the results of this study, a dosage regimen for intranasal vitamin B12 administration has been proposed comprising a combination of the loading dose and no loading dose regimen, with administration every 3 days for 2 weeks followed by weekly administration.
