Provisional publication of Never Events reported as occurring between 1 April and 31 December 2019

There were 350 serious incidents including 1 enteral medication given IV, 4 oral meds given IV, 2 oral meds given subcut, 6 methotrexate overdose prescribed and administered, 1 insulin withdrawn from insulin pen, 3 wrong strength midazolam and 1 wrong strength potassium given.
