Severe and fatal medication errors in hospitals: findings from the Norwegian Incident Reporting System

Review of 3372 medication errors reported between 2016-2017 found that most errors occurred during administration (68%), followed by prescribing (24%). Dosing errors were the most common (38%) followed by omissions (23%). 5.2% caused severe harm and 0.8% were fatal.
