Severe Exacerbations of Systemic Capillary Leak Syndrome After COVID-19 Vaccination: A Case Series

Letter describes 3 patients with flares of systemic capillary leak syndrome (SCLS) 1-2 days after receiving Covid-19 vaccine (Janssen, Moderna, Pfizer-BioNTech), in whom no other triggers were identified. Authors recommend IVIG prophylaxis prior to vaccination in those with SCLC.

SPS commentary:

In systemic capillary leak syndrome, plasma leaks into peripheral tissues, which typically leads to hypotensive shock and multiple organ dysfunction. It is an extremely rare relapsing-remitting condition and triggers for relapses are not well understood.

As of 6th October, the MHRA had received 12 reports of capillary leak syndrome in the context of more than 48.9 million doses of COVID-19 Vaccine AstraZeneca given. Of these reports, 2 people had a history of capillary leak syndrome. As a precautionary measure, the MHRA is advising that COVID-19 vaccine AstraZeneca is not used in people who have previously experienced episodes of capillary leak syndrome.
