The nature, severity and causes of medication incidents from an Australian community pharmacy incident reporting system: the QUMwatch study
Study (1013 incidents reported over 30 months, 831 near misses; 165 involved patient harm) found largest incidence in >65 year olds (35.7%); most involved errors during prescribing (61.1%) and with systemic antibacterials, analgesics and renin-angiotensin medicines.
