Ultra-early tranexamic acid after subarachnoid haemorrhage (ULTRA): a randomised controlled trial

RCT (n=955) reports that in patients with CT-proven subarachnoid haemorrhage, ultra-early, short-term tranexamic acid treatment (<72 h) did not improve clinical outcome at 6 months compared to usual care only; as measured by the modified Rankin Scale.

SPS commentary:

Authors’ of a related commentary highlight that this RCT, which evaluated faster initiation and shorter duration of tranexamic acid (<72 h) along with early aneurysm treatment, does however suggest tranexamic acid might reduce the risk of re-bleeding without increasing the risk of cerebral ischaemia; but without improving functional outcomes.
